I bet that made the Doctor feel awful because he changes so much on the outside, and knew so many Time Lords who did too, that he must identify people by their selves, not their looks.

He could tell when Rose was Cassandra.

He could tell when Amy was a Flesh.

He sees who people really are. That’s why he loves them so much.
